K replacement in DKA

Note that

  • We should NOT give >20 mmol/hour of potassium (1.5g of KCl) WITHOUT cardiac monitoring
  • If PO or enteral route is available, use it.
    – K 3.5-5: 20 mmol
    – K < 3.5: 40 mmol
    1 tab of slow K (oral KCl) = 8 mmol of K (2.5 tabs if K = 3.5-5 and 5 tabs if K < 3.5)
    1 mL of KCl syrup = 2 mmol of K (10 mL gives 20 mL, 20 mL gives 40 mL)
